The Application Process

Welcome to the Humane Society's adoption program. The information on the following pages is requested so that we can assist you in the selection of a new pet. These forms and consultation with a Humane Society representative are designed to help us determine if the adoption is in the best interest of the animal; and to assist you in finding an animal most compatible with your lifestyle.

In order to be considered as an adopter you must:

Have a 6 ft. totally fenced yard. Some exceptions can be made for dogs less than 20 lbs.
A home visit is required prior to adoption.
Have the knowledge and consent of your landlord if you rent
Be able and willing to spend the time and money necessary to provide training, medical treatment, and proper care for a pet.
No one living in the house has any criminal convictions that portrays aggression to an animal or a person
Be 18 years of age or older (unless employed full-time and responsible). If living with a parent, relative or friend, the agreement of that person/people that you can have a pet.
Have photo identification showing your present address
You do not live in a town or county where a pit bull, or other breed of dog, is considered a dangerous dog due to its breed rather than what it has done.

The Humane Society retains the right to refuse to adopt an animal to any person where they feel the adoption would not be in the best interest of the animal.

Note that the form contains two sections -- the application form and the contract form. PLEASE FILL OUT BOTH FORMS. You may add any additional information on a separate sheet(s).
